I have taken it on an empty stomach, first thing in the morning and had no problems with it. Then again, when I have had a bad case of heartburn, after eating, I have taken it to help with that.
To help with digestion, I would say it is better before you eat your meal.
I guess it would depend on the 'whys' or what you feel most comfortable with. (Sounds like sitting on the fence here, huh :D)

This is a reply to # 946,276I like to take it 10-20 minutes before I eat. I take mine is 1/2 C of applesauce. I used to take it it water, but like it better masked with the applesauce.

This is a reply to # 946,276I combine it with honey and take it before or after I eat once or twice a day.
I had high cholesterol three months ago. I started taking vytorin and ACV every day. My cholesterol and triglicerides are normal now.
I stopped the vytorin, but I am still taking the ACV. I'll see my doctor 3 months from now. I'll let you know what happens.- Re: Best way to take ACV #35567

Consuming it in warm water made me extremely queasy.

This is a reply to # 943,604ACV is extremely acidic. So I would not take it on empty stomach. I would take it after eating a meal. Like 2 tbsp in a glass of water. That way it will help with digestion of food.
Alternativly, u can take freshly squeezed lemon in a glass of water after eating your food.
The extra acid will help with digestion.
Taking ACV on empty stomach may make your stomach hurt, because u just reduced the acidity of the stomach, when it was not needed.
