a couple things come to mind

insufficient bile flow (culprits can be mercury, copper, lead, parasites, among many other things)

however your stools would give you a big clue if this is the case.

mitochondrial problems (mercury is a common culprit) will cause problems with metabolizing fat.. a trial of mito-cocktail is worth trying.

if your testosterone and dhea are oral, they can be hard on your liver if your liver is not healthy.
