In case you've tried everything else, here are some things to look in to:
1) McCombs plan - candida plan. If you google it you will find it. It really does work if candida is your problem.
2) Check your Iodine levels - Doctor's Data makes a test for this.
3) Check your heavy metals. Doctor's Data also makes a test for that - the 6 hour Urine heavy metals challenge test. Heavy metals can also cause persistent candida, among many other things like constipation, etc.
