Visit with Dr. Cahill parasite itch

I just saw Dr Cahill recently first time-- for 4 yrs of rectal itching that migrated to nose and eyes two years ago. Took Vermox (mebendazole) 2 years ago that relieved symptoms immediately but the symptoms returned after medication finished. I haven't received an official report from Cahill, but I have been prescribed Tinidazole 1000 mg for 3 days, repeat in 10 days for "pus and crystals" found in rectal exam. Also rectum noted to be reddened. I also have dry skin that looks like wrinkled paper in some areas and a rash on my breasts. Also to note I have had some weight loss, abdomen distension after small amount of food, increased gas at night and lots of gurgling on right side, waking in the middle of the night, increased rectal and nasal itch crawling even as I type.
What are your thoughts? I am not sure why I wasn't prescribed mebendazole or albendazole. I want to start at least a month long regimen of medication to put an end to this. I can obtain ivermectin now but want pill form of albendazole, mebendazole and probably an antibiotic. Where should I order from? If someone can advise on a reputable pharmacy I really need to place an order soon. Should I bother starting the Tinidazole (Tindamax) if I will not be taking ivermectin or mebendazole or albendazole as well at the same time?
