Persistent Vitamin A Deficiency since 2 years

Dear all,
Since there is so little info on vitamin A deficiency to find online, I decided to ask the question here.
The thing is, I've been deficient since 2 years if not more, I only stared testing it 2 years ago.
Since then, I've tried Nordic Naturals CLO capsules and liquid, Bluebonnet Vitamin A and Country Life Vitamin A. Last two contained mainly retinol and beta carotene. I just ordered Carlson Lab Vitamin A+D, which is pure fish oil derived.
Even though I've done all this for 2 years, including a paleo diet with lots of chicken livers, foie gras, egg yolks, german grass fed butter, heavy cream and such, my Vitamin A keeps coming back at 250 while the range is 300-1200.
Because I have several auto immune issues, I'm aware that Vitamin A plays a very important role among other vitamins and minerals such as Vit D+E+K and Zinc and Iodine. I was also deficient in all of those but have been able to get them in the higher/normal range and keep them there.
What could be the problem with me not being able to increase Vitamin A levels? Is it malabsorption? If so, then why did all the other fat soluble vitamins increase after supplementation?
I'm done all I could in terms of research and am not sure what it could be.
Also, I'm almost 2 months pregnant now and since my first pregnancy was a miscarriage, I'm trying to do everything I can to make sure my baby sticks this time :) (Yes, yes, progesterone, cortisol, etc are all taken care off this time around :) )
I really appreciate it if anyone would give their thoughts on this, as I don't know who to turn to. Of course my doctor doesn't want any of this, since doctors believe Vitamin A during pregnancy can cause birth defects. But they should actually also be aware of the fact that Vitamin A deficiency can cause such things as well!!
