dehydration. environmental triggers. physical injury. but yea, metabolic disease is the real culprit. cats and dogs didnt used to get cancer either until we started feeding them grains for food. we are ancestrally carnivores.

edit: i believe the smoking connection is more of an alcohol connection, if there ever was one - again, fatty liver, dehydration and insulin resistance.
