Molybdenum: brand -Health Yourself- 250mcg per tablet. I only take one per day. This is a little shy of what the article suggested, and if I feel like I have a lot of die-off sometimes I take two. Not the "divided doses" they suggested, but so many supplements say that and hey, I have to work! :)
Pantethine Complex: brand- the Vitaminshoppe - 150mg each of pantothenic acid and pantethine in each tablet. I vary my dosage within the range suggested in the article. When I feel well, I still take the 450 mg. Days like today, when I can tell I hit those antifungals heavy yesterday, I'll have a full 900 mg.
