This looks like what I would call mucoid plaque. I have seen these come out, I called them starfish. I know others have seen these. It is a build up of whatever because of a gi infection probably created by the immune reactions. Shroom had the name for this.

It looks like no worm or fluke that I have seen. You could take one of those (in preservative) to a lab for identification.
